Job Description

General Statement of Duties:
Provides culturally competent and appropriate behavioral health treatment for assigned clients. Provides counseling and education for individuals, couples, families and groups. Coordinates behavioral health services with other community and State resources. Does related work as required.

Supervision Received:
The QMHP is supervised by Outpatient Mental Health Director with input from Clinical Director.

Supervision Responsibility:
There are no supervisory responsibilities for this position.

Essential Duties & Responsibilities:
Conducts comprehensive and diagnostic evaluations using DSM V criteria; develops treatment plans which may include the delivery of medical, gambling, substance abuse and/ or mental health services to include prescribed medications and the maintenance of medication supplies.
• Provides initial assessment and service plan upon intake.
• Coordinates with Intake Specialist and Program Director for assigning individuals for appropriate long term service.
• Provides referrals for substance use treatment, gambling treatment or other specialty service.
• Provides individual and group therapeutic interventions for individuals and their families
• Participate in team meetings and coordinate all activities with multi-disciplinary team
• Carries a caseload as appropriate
Other Duties:
• Maintain appropriate certifications and licensing for appropriate licensing boards in the State of Oregon.
• Governed by a code of ethics of the individual’s professional discipline including rules of confidentiality; working knowledge and techniques of comprehensive, community mental health services and the relationship of all mental health disciplines to the field of community of psychiatry.
• Skill in dealing with mentally ill, developmentally disabled and alcohol or drug addicted persons and able to understand the behavior of people under stress.
• The ability to develop and maintain cooperative relationships with patients, their families, and the community.
• Responsible for maintenance and timeliness of all records (i.e. mental health assessments, treatment plans, progress notes, level of care instruments, etc.), and other required documentation.
• Assists and/or provides back-up support with screening, referral, and crisis services as needed and directed by supervisor.
• Participates in team meetings as scheduled or assigned.
• Must have the ability to communicate effectively in writing and verbally.
• Must interact amiably with co-workers, clients and outside contacts.

Minimum Qualifications:
• Must have a Masters in Social Work, Counseling or related field. Preference for LCSW/LPC or working towards professional licensure. Satisfy QMHP requirements as spelled out by Oregon State Statute – Department of Human Services Addictions and Mental Health Division-Integrated Services and Supports Rule 309-032-1520 Personnel (3) (f).

Preferred Qualifications:
• Journey-level professional counseling position with an MA/MSW degree plus two years of experience or equivalent combination of education and experience.

Work Environment:
Work shall be performed at New Directions Northwest, Inc.’s outpatient facility and in the community as necessary. Majority of the work will be completed indoors in an office environment. On occasion travel will be required to other facilities. This position requires the employee to dress in a professional manner and to wear attire appropriate for the position.
